November 18, 2018, Alex Smith nearly lost his leg after suffering a spiral compound fracture to his tibia and fibula, an injury that ended the career of Redskins legend Joe Theismann as well. Many people, including the former number one overall pick of the 2005 draft, questioned whether or not Smith would make a return to the gridiron.

“I’m completely helpless,” Smith said in ESPN’s E60 documentary on his rehab, Project 11. “I can’t get up by myself, I can’t do anything. Am I ever going to be normal. Functional? [I was] really having a strong doubt in my head that that was going to be a reality.

ESPN recently aired an E60 documentary focused in on Alex Smith’s rehab and recovery from his gruesome leg injury called Project 11. The documentary takes all the way from his playing days in Utah to his injury in Washington in 2018. It further details his battles with post-operative infection, his intense recovery and rehabilitation, and his determination to play in the NFL again.

Smith’s wife, Elizabeth, pinned an encouraging workout video of Alex to her instagram to celebrate his 36th birthday. In the video, Alex is sprinting, working through his progression, making accurate throws and most notably, putting a lot weight on his surgically-repaired leg.

“Happy Happy Birthday to this handsome, hard working man,” Elizabeth wrote on Instagram. “The last week we’ve been overwhelmed with love, support and sweet messages. We cannot thank you enough. #project11 was a look into what the last 18 months have been (mostly Alex refusing to give up). I am in awe of his perseverance everyday. He just keeps getting stronger and refuses to give up…Forever grateful.”
